Why was my paper not published in the proceedings despite being accepted?

There are multiple reasons why an accepted paper could not be published.

The most obvious reason is that the author never registered the paper with the correct package and thus never uploaded the final Camera-ready version.

In the case that a paper has been turned into the camera-ready version, it is still possible that the publisher will reject a paper for being out of scope of the book series / journal.
